Financial Disclosure
TAMPA, Fla. (May 7, 2009) -- Each year, employees filling certain designated positions must complete a Statement of Financial Interests Form and submit it to the Department of State in Tallahassee by July 1.
USF has approximately 75 positions so designated. The forms are mailed directly from the state to the incumbents of the designated positions. Affected individuals should receive the form and the guide during the month of May.
The forms must be completed, signed and returned to Tallahassee even if none of the items apply (retain a copy of the completed form for your records). Please do not send the completed form to Human Resources. The mailing address is listed on the back of the form. For more information, contact Nancy Ward in Human Resources at (813) 974-5385.
