Please download and read the Patel Grand Challenge Governance Document (pdf).

Once you have confirmed your eligibility, download the Template for Proposal Submission (TPS) (pdf), fill it out completely and upload below per instructions by April 2, 2012 (midnight EST). Only one TPS per group is required by the principal applicant.

You will also be required to submit the following additional documents:

Principal applicants submitting on behalf of a group must submit forms on behalf of all partners in single PDF documents. That is, one PDF document containing all W-8 IRS tax forms another PDF document containing all letters of consent and a final PDF document containing all CVs. When scanning documents or images to PDFs, please use no more than 128dpi (dots per inch).

Applicants will be required to provide a W-8 BEN form as the award may be subject to U.S. taxes. The information required in this form is for U.S. tax reporting purposes as required by the Internal Revenue Code under sections 3402(f)(2)(A) and 6109 and their regulations. Social Security Numbers are not being collected by the University of South Florida.
